Our center provides post-acute rehabilitation and long-term care services, including a myriad of medical and therapy services. We offer around-the-clock care for those living with chronic illnesses and disabilities. Each day, our skilled, compassionate team strives to help our guests become a better, brighter, and stronger version of themselves.

Through our extensive network of National Health Care centers and affiliate partners, we offer access to a continuum of post-acute services including therapy, telehealth, diagnostics, home care, and hospice.

Our Impact

My father just had a short stay at Maefair Health Care Center for post-hospital care focused on physical rehabilitation, and I was extremely impressed with the facilities, rooms, and staff. I live across the country and was able to visit for a few days. They took such good care of my father while he was there. He arrived using a walker and his rehab was so successful that he walked on his own out of the center on his last day. I felt that they were very attentive to his needs both mentally and physically. In addition, everyone that I came in contact with was very pleasant, genuine, and welcoming. I was met with such a warm hello upon entering the facility on the first day. I want to thank all of the staff for their help, and in particular the social worker, Paige. She always kept my family and I in the loop about my dad’s progress and assisted us greatly with his discharge plans. Living out of state it was very reassuring to work with someone who so competent and knowledgeable about the different resources that my father was able to take advantage of. In addition, she was very patient with me as it was hard to get a hold of me at times while I was at work and with the time difference. She also went above and beyond to steer me in the direction of other resources that my mom might be able to access after she was discharged from the hospital (my parents were both in the hospital at the same time), even though she was not a patient at Maefair.

My mother has been there for a while now and she’s always been kept well. Thank you to the staff for always being respectful and treating my mother and my family well. There’s moments like anywhere else where everything isn’t always the greatest but on a scale of one to ten she’s happy. No bed sores, no dirty sheets that I notice when I visit. Whatever she asks for she receives and whatever we her family asks we receive. Thank you Maefair for making my mothers permanent stay here be well and comfortable.

My sister-in-law was a Maefair resident for the past year and I can't rave enough about the excellent care she received. The staff is very dedicated and professional. They remained in constant contact with me regarding her health, especially during the Covid pandemic. The recreation department is top notch, they offer many varied activities. The Christmas party was incredible and I could see how much all the residents appreciated being able to invite their families to attend.
